Same-day implants are not for every patient. They require sufficient bone density at the implant site, a healthy gum line, and a low-force bite location (usually front teeth or premolars rather than molars). How immediate-load single-tooth implants work — and why approximately 30–40% of patients are not candidates regardless of what the marketing says.

The biomechanics of immediate-load single-tooth implants

A same-day implant — known clinically as 'immediate provisionalization' or 'immediate loading' — places the titanium fixture into a fresh or healed extraction site and immediately seats a non-occluding temporary crown. The key word is 'non-occluding': the temporary crown is intentionally shaped so it does not contact the opposing tooth during normal chewing. Functional loading during the 3–4 month osseointegration phase causes implant micromotion, and micromotion above approximately 150 microns reliably disrupts the bone-implant interface and produces implant failure. For immediate provisionalization to succeed, three conditions must be met: (1) the implant must achieve at least 35 Ncm of initial insertion torque, demonstrating primary mechanical stability; (2) the temporary crown must be designed out of occlusion; (3) the patient must comply with soft-diet restrictions for the first 6–8 weeks. Without all three, the same-day temporary crown becomes a leading cause of implant failure rather than a convenience benefit.

Who is actually a same-day implant candidate: clinical criteria

Roughly 60–70% of single-tooth implant candidates at our practice are appropriate for the same-day immediate-load protocol. The remaining 30–40% fall outside the criteria for one or more reasons: insufficient cortical bone thickness at the planned implant site (visible on CBCT, often in posterior maxilla where the alveolar bone has thinned post-extraction), Lekholm-Zarb type IV (low-density) bone that will not generate the required 35+ Ncm initial torque, acute periapical infection at the extraction site requiring debridement and a healing period before fixture placement, heavy parafunctional activity (bruxism with documented attrition) that makes maintaining the non-occluding temporary configuration unrealistic, or molar sites where the lateral cantilever forces during incidental food contact exceed the immediate-load safety margin.
